Private Home Care Provider - Application

The Private Home Care Provider (PHCP) Program is regulated by the Georgia Department of Community Health (DCH), Healthcare Facility Regulation Division (HFRD). This program licenses and monitors private home care agencies to provide in-home personal care, companion/sitter, and nursing services to clients in private residences across Georgia.

Price starts at $4,500.
